NOTICE OF OFFICE CLOSURE EFFECTIVE DEC 22, 2025
I am informing you that the office is closing Dec. 22, 2025 as I am retiring Dec. 31, 2025. Another physican will not be taking over the practice.
If you would like access to your records before Dec. 22, 2025 you may leave a message or you may reach out over the portal anytime and download your entire medical record yourself from the portal. If you would like your record faxed to another provider, please fill out the medical release form under the forms section of your portal and upload the signed form. We will let you know when your records have been sent. Requests for records to be mailed directly to you are charged $25.
Prescription refills will be provided for one year from your last appointment or until you find a new provider. Refill requests should be made over the athena portal. This can be reached through the athena portal website at dwootenmd.com or this website wootenmd.com

You may also choos any gynecologist of your preference.

We are located in the large brick building at 101 Court St in Plymouth, MA. You can enter the building from the backside where the parking lot is located. Please dial "08" in the intercom system to be let into the building. Dialing directions are located by the intercom system.
You may take the elevator or use the stairs up to the 4th floor. Once arriving on the 4th floor, head to your right down the corridor. There you will see a sign for the office.
Parking: There is a small parking lot behind the building for your convenience. There is also metered parking along S Park Ave, which runs along the side of the building. Please call our office if you are having difficulty finding a parking space.
